We hold non-medical Personal Assistance Services in our lane and coordinate with the clinical team that owns the rest. Our standing referral relationships include:

Turnaround: built for discharge timelines
Call us before the discharge order is written, not after. We can meet the patient and family at the bedside, complete our assessment in the hospital or facility, and have a caregiver in the home the day of discharge. Same-day and next-day starts are routine; when a start time is not realistic, we say so on the first call rather than leaving your patient uncovered.
Two policies make us practical for bridging plans. First, our minimum visit is four hours — one block that covers the highest-risk window on discharge day: meals, hygiene, medication reminders, and a safety check, all at one flat hourly rate, with care often in place within 24 hours. Second, we have no "levels of care" pricing: the family's rate does not increase as acuity increases, which removes a common reason placements fall apart at 60 days.
Where we provide care
We staff caregivers in private residences, hospitals, and healthcare facilities. That includes sitter coverage in hospital rooms, one-on-one caregivers inside assisted living, independent living, and memory care communities, and support alongside hospice and Medicare home health in the home. We are a scheduling and daily-living layer for your clinical plan — we do not provide skilled nursing, wound care, injections, or any medical treatment, and we do not represent otherwise to families.

Caregivers document each shift against the written care plan, and we keep records available to the family and, with the client's consent, to the referring professional. If a client's condition changes in a way your team should know about, we flag it — we are often the only set of eyes in the home between your visits.
For clients paying through long-term care insurance, we prepare and submit the claim paperwork and ongoing invoices the carrier requires. LTC insurance agents and elder law attorneys refer to us for this reason: benefits their clients paid decades for actually get used.
What you will not get from us: scope creep. We do not market skilled services we are not licensed for, we do not tell families we can replace home health or hospice, and we do not quietly re-tier a family's rate after you vouched for us. Your referral is a professional risk on your part; our job is to make it a safe one.

- Give us the picture Discharge date, address, hours needed, mobility and cognition, and who holds decision-making authority. Five minutes of detail lets us quote the family accurately on the first call.
- We take it from there We contact the family the same day, complete a free assessment at bedside or at home, and confirm the start with you once care is scheduled.
